Accessing Your Child's or Dependent's My Health Account

It is important for parents and legal representatives to have access to their child's/dependent's health information. We can help you set up this access. Only parents and legal guardians** can have access.

You must have your own My Health at Vanderbilt account before accessing a child's/dependent's account. If you are currently a patient, you can create your account at myhealthatvanderbilt.com . If you are not currently a patient, you will be sent a sign-up link once we associate your child/dependent to your account.

Ask staff for an application, or download a printable version of the age-appropriate form below, complete it, and bring it with you to one of the following locations:

Once the application has been processed by the My Health at Vanderbilt Team, you can then access your child’s/dependent’s account by logging into your account and clicking on the patient’s name.

**For representatives other than parents, please provide legal documentation to the clinic or fax to (615) 875-2820. This includes: Custody, Health Care Power of Attorney, Conservatorship, Guardianship, Department of Children Services Letter for Foster Parents, etc. You may also submit documentation via secure email to MHAVLegal@vumc.org. Please note that email that is not sent securely (i.e., unencrypted email) may be intercepted and seen by others during transmission. By choosing to send information by unencrypted email, you accept these risks.
